Congratulations you are getting married or have recently wed! Now it is time to go through the process of changing your last name.
Step 1: Marriage License
You will need a certified copy of your marriage certificate. Make sure to request a copy. It general takes between 4-6 weeks for deliver by you can have it expedited. Using a uncertified copy (memento) will not be accepted.
Step 2: SOCIAL SECURITY OFFICE
Provide the Social Security office your Birth Certificate, Government Identification & Certified Marriage Certificate. They will begin to process of your name change that shouldn't take longer than a week. You should receive proof of your successful name change.
STEP 3: Receive Social Security documents confirming your name change
You must receive a confirmation letter from the Social Security Office before going to the DMV to change your Government ID
Step 4: Department of Motor Vehicles (DMV)
You can change your name on your government issued license, car titles and registration as long as you present your document from Social Security with proof of name change, your government ID, Certified Marriage Certificate, Social Security Card and proof of address.

Credit Union & Bank Accounts /Retirement & Investment Accounts
Credit Cards and Rewards Cards
Vehicle (Car) Loans or Leases
Insurance (Vehicle, Home, Medical,Life)
Utilities (Gas, Electric, Water/Sewer/Garbage, Phone, Internet/Cable
